ABSTRACT

Objective To analyze the clinical characteristics and regularity of aristolochic acid nephropathy (AAN) induced by drugs containing aristolochic acid. Methods The clinical data of 111 patients with AAN induced by aristolochic acid were reviewed. The clinical features, medication and treatment of AAN were analyzed. Results Among 111 patients, there were more females than males (2.58∶1), 101 cases (90.99%) were over 50 years old; the mean age was (63.70±11.67) years old;the average duration of medication was (8.08±6.94) years. The drugs involved were Guanxinsuhe pill and Longdanxiegan pill in 106 cases (95.50%). Serum creatinine increased in 108 cases, urea nitrogen increased in 106 cases and hemoglobin decreased in 103 cases, most of which were hypogravity urine, mild to moderate proteinuria and occult blood. Ultrasonic examination revealed that the kidneys were damaged to varying degrees. Pathological biopsy of kidney showed renal tubular damage. Most patients had an insidious onset and varying degrees of progression, which were not proportional to the age and the duration of taking the medicine. In clinical, the renal function was progressively damaged, most of which were irreversible and with a poor prognosis. Conclusion Patients with renal impairment differed greatly individually, and the renal damage was not paralleled with the medication duration and dose of drugs containing aristolochic acid.AAN progressed rapidly, and the disease still progressed even after stopping taking drugs containing aristolochic acid. Strengthening pharmacovigilance, implementing early diagnosis and effective intervention could help to reduce the occurrence of AAN and attenuate its development.

ABSTRACT

To investigate the effect of long-term indwelling gastric tube on the prevention and treatment of esophageal stenosis after endoscopic submucosal dissection (ESD) for esophageal circumferential superficial cancer, data of patients with esophageal circumferential superficial cancer who underwent ESD in the First Affiliated Hospital of Nanjing Medical University from January 2018 to December 2021 were retrospectively analyzed. There were 15 patients with gastric tube placement (GTP) after ESD (the GTP group ), and 23 patients without GTP (the non-GTP group). The general information, lesion location, pathological stage, postoperative complications, degree of esophageal stenosis (water intake), pain conditions, number of hospitalizations and medical expenses were compared between the two groups. The results showed that there was no significant difference in age, gender, lesion location or postoperative pathological stage between the two groups ( P>0.05). Compared with the non-GTP group, the rate of water intake in the GTP group was significantly higher (11/15 VS 6/23, P<0.05), the frequency of pain was less in the GTP group (7.3±3.1 times VS 10.7±3.6 times, t=3.00, P<0.05), and the number of hospitalizations and the medical expenses after ESD to before and after stent placement were significantly lower in the GTP group than those in the non-GTP group ( P<0.05). There were no significant differences in the incidence of delayed bleeding and perforation, or time of the first stenosis after ESD between the two groups ( P>0.05). The results of the study initially showed that long-term indwelling gastric tube after ESD can reduce the degree of esophageal stenosis with good safety for esophageal circumferential superficial lesions.

ABSTRACT

Objective:To explore the values of dynamic 18F-FDG PET/CT semi-quantitative parameters in the differentiation of histological subtypes of non-small cell lung cancer (NSCLC) and the diagnosis of lymph node metastasis. Methods:Twenty-three patients (10 males, 13 females, age (61.5±8.1) years) with NSCLC in Shengjing Hospital of China Medical University were prospectively enrolled between October 2020 and June 2021. All patients underwent 40 min of dynamic PET/CT scan and static scan at 60 min post-injection of 18F-FDG. SUV max, SUV mean, metabolic tumor volume (MTV) and total lesion glycolysis (TLG) of primary lesions at different stages of PET/CT imaging were evaluated. SUV max of lymph nodes were also analyzed. The histopathological results were considered as the gold standard. Parameters of primary lesions and lymph nodes at different PET/CT imaging stages in different groups were compared by independent-sample t test or Mann-Whitney U test. The diagnostic efficiencies of those parameters were tested by ROC curve and compared by DeLong test. Results:A total of 24 lesions in 23 patients were pathologically confirmed as NSCLC, of which 11 were squamous cell carcinoma and 13 were adenocarcinoma. The SUV max (13.5±3.4 vs 9.6±5.1), SUV mean (8.1±2.2 vs 5.8±3.2) at the third stage of dynamic PET/CT (33-40 min) between squamous cell carcinoma and adenocarcinoma were both significantly different ( t values: 2.20, 2.10, P values: 0.039, 0.048). SUV max, TLG of static PET/CT between the 2 groups were also different ( t=2.22, P=0.037; z=-2.17, P=0.030). ROC curves of those parameters showed that AUCs were between 0.727 and 0.762. Fourteen patients underwent surgery and lymph node dissection. According to the pathological results, lymph nodes in 15 areas were metastatic, the rest in 70 areas were benign. There were significant differences in SUV max at the second stage of dynamic PET/CT (19-26 min) between metastatic and benign lymph nodes (4.0(2.8, 6.2) vs 2.3(1.8, 2.8); z=-4.31, P<0.001), as well as SUV max at the third stage of dynamic PET/CT and static PET/CT between the 2 groups ( z values: -4.59, -4.10, both P<0.001). ROC curves of those 3 parameters showed that the AUCs were 0.856, 0.879 and 0.838 respectively, with no significant differences ( z values: 0.78, 0.34, 1.27, P values: 0.434, 0.734, 0.205). The sensitivity, specificity, and accuracy of the SUV max of the third dynamic imaging stage were 13/15, 90.0%(63/70) and 89.4%(76/85), respectively. Conclusion:Dynamic 18F-FDG PET/CT semi-quantitative parameters have certain clinical significance in the identification of histological subtypes of NSCLC and the diagnosis of metastatic lymph nodes, especially the third stage dynamic imaging has a better diagnostic performance.

ABSTRACT

ObjectiveBased on the antidepressant activity, the optimal ratio of Rehmanniae Radix and Lilii Bulbus in Baihe Dihuangtang was optimized and its possible mechanism was preliminarily explored. MethodA total of 100 male mice were randomly divided into normal group, model group, Rehmanniae Radix single decoction group, Lilii Bulbus single decoction group, and 5 different ratios of Baihe Dihuangtang group (Rehmanniae Radix-Lilii Bulbus 5∶6, 5∶5 , 5∶4, 5∶3, 5∶2), fluoxetine hydrochloride group. Except for the normal group, the remaining 9 groups were treated with chronic unpredictable mild stress method (CUMS) for 21 consecutive days to establish the depression mouse model. The Rehmanniae Radix single decoction group, Lilii Bulbus single decoction group and 5 different ratios of Baihe Dihuangtang group were administered with corresponding drugs at a dose of 2 g·kg-1 respectively, and fluoxetine hydrochloride group was administered fluoxetine hydrochloride at a dose of 0.01 g·kg-1, the normal group and the model group were respectively given an equal volume of normal saline by gavage for 21 consecutive days. The antidepressant effect of Baihe Dihuangtang with different ratios was evaluated by behavioral experiments. Western blot was used to detect the expression levels of brain-derived neurotrophic factor (BDNF) and tyrosine kinase receptor B (TrkB) in hippocampus. The level of total superoxide dismutase (SOD) in hippocampus was detected by hydroxylamine method. The level of reduced glutathione (GSH) in hippocampus was detected by spectrophotometric method. The levels of glutathione peroxidase (GPX) and glutathione S-transferase (GST) in hippocampus was detected by colorimetric method. The level of malondialdehyde (MDA) in hippocampus was detected by thiobarbituric acid method, and preliminary exploration of its antidepressant mechanism. ResultCUMS caused a significant decrease in the preference rate of sugar water and a significant prolongation of forced swimming and tail suspension immobility time (P<0.01), while the different ratios of Baihe Dihuangtang and the single decoction of Rehmanniae Radix and Lilii Bulbus were significantly reversed to varying degrees. Abnormalities of the above indicators were observed (P<0.05, P<0.01), especially when the ratio of 5∶6 had the most obvious effect on the above indicators, Moreover, CUMS caused the hippocampal BDNF and the expression of TrkB, SOD and glutathione antioxidant-related indexes were significantly decreased (P<0.01), and the levels of lipid peroxidation product MDA was significantly increased (P<0.01). However, the superior ratio of Baihe Dihuangtang (Rehmanniae Radix-Lilii Bulbus 5∶6, 5∶5, 5∶4) and the single decoction of Rehmanniae Radix and Lilii Bulbus significantly reversed the abnormality of the above indicators to varying degrees (P<0.05, P<0.01). ConclusionIn summary, 5 different ratios of Baihe Dihuangtang and the single decoction of Rehmanniae Radix and Lilii Bulbus can produce antidepressant effect, especially when the ratio of 5∶6 had the most obvious effect on the above indicators, and the antidepressant effect gradually weakens with the proportion of Lilii Bulbus in the recipe decreases, and its mechanism may involve enhancing the nutritional level and antioxidant defense ability of hippocampal central nervous system.

ABSTRACT

Objective@#To predict the tumor neoantigen peptides in hepatocellular carcinoma (HCC), and examine their specific immune effects against the tumor cells without injury to normal cells.@*Methods@#The data of whole-genome sequencing and exome sequencing of HCC tumor and matched non-tumor liver tissues were analyzed to confirm the HCC-associated somatic mutations. Based on the HLA phenotype of the patients, we used NetMHC software to predict the neoantigen epitopes with high binding affinity to their MHC-I molecules. The predicted peptides with mutation sites included were synthesized. GPL10687 platform was applied to examine the gene expression difference between tumor and normal tissues of the selected genes in GSE25097, one of the GEO databases. The quantitative real-time PCR (qRT-qPCR) and immunohistochemistry were used to confirm the expressions in tumors and normal tissues of the selected genes. By using the predicted peptides, we induced the generation of antigen-specific CD8+ cytotoxic T lymphocytes (CTLs) and examined their specific effects against tumor cells.@*Results@#The mutation frequency of TP53 (tumor protein p53) was 40%, and LAMA3 (Laminin Subunit Alpha 3) was 8% in the analyzed HCC tissues. In GSE25097 database, TP53 and LAMA3 mRNA levels in tumors were 1.57±0.02 and 1.37±0.10, which were significantly increased than those in matched no-tumor tissue (0.54±0.01 and 0.36±0.01, P<0.05). The differences of expression levels of TP53 and LAMA3 in tumor and no-tumor tissues were validated by using qRT-qPCR and immunohistochemistry in 10 HCC tissues. The mRNA levels of TP53 and LAMA3 in tumors were 0.24±0.03 and 0.13±0.06, which were significantly elevated than those in matched no-tumor tissue (0.11±0.01 and 0.01±0.01, P<0.05). Among the Chinese population, HLA-A2 and HLA-A11 and HLA-A24 accounted for 70%, representing the major MHC-I molecules. The CTLs induced by predicted peptides showed cytotoxicity to the targets pulsed with mutated peptide, with no effect on the target pulsed with normal peptide and on normal cells.@*Conclusions@#TP53 and LAMA3 existed relative higher mutation frequency in HCC, and expressed higher in tumor tissues. The induced CTLs by predicted peptides derived from mutation-associated protein could specific kill the target cells without injury to normal cells.

ABSTRACT

Objective To analyze the relationship between sleep duration, screen viewing time, and the prevalence of overweight/obesity among primary school students in Xuzhou. Methods Using a cluster sampling method, a total of 3 228 students (including 1 679 boys and 1 549 girls with an average age of 10.78±0.69 years) from grade one to six from 10 primary schools in Xuzhou underwent interview using a self?designed questionnaire containing basic characteristics, sleep duration, and screen viewing time. Data on height and weight were also collected. The relationship between sleep duration, screen viewing time, and overweight/obesity was analyzed using a logistic regression analysis. Results The prevalence rates of overweight among boys and girls were 16.56% and 11.94%, respectively (χ2=13.59, P<0.05). The prevalence rates of obesity among boys and girls were 14.47% and 10.07%, respectively (χ2=14.01, P<0.05). In total, 74.41% students reported a lack of sleep; the average sleeping time was (9.24±1.07) h. The average sleeping time among boys was (9.35 ± 1.12) h and among girls was (9.13 ± 1.03) h. The difference in sleep duration between boys and girls was significant (t=5.79, P<0.05). The differences in sleep duration and overweight/obesity were significant between both boys (χ2=18.62, P<0.05) and girls (χ2=21.14, P<0.05). Regarding screen viewing time, 17.29% of students spent more than 2 hours per day viewing a screen. The difference in screen viewing time between boys and girls was significant (Z=3.02, P=0.014). The proportion of children with screen viewing time of more than 2 h/d among overweight/obese and healthy weight male students was 29.50% (82/278) and 22.56% (316/1401), respectively, which was significantly different (χ2=6.18, P=0.01). However, there was no significant difference when examining the same groups among girls (12.98% (24/185;obese/overweight) vs . 9.97% (136/1364; healthy weight); χ2=1.59, P=0.21). After adjusting for parental obesity, eating sweets, and physical activity, logistic regression analysis showed that students who had a sleep duration less than 10 h/d had an odds ratio of 1.4 (95% CI: 1.15-1.71), the odds ratio for boys and girls was 1.56 (95% CI: 1.13-2.14) and 1.69 (95% CI: 1.15-2.46). The students who had a screen viewing time of more than 2 h/d had an odds ratio of 1.14 (95% CI: 1.05-1.80); the odds ratio for boys in this group was 1.18 (95% CI: 1.03-1.67). Conclusion Short sleep duration is a risk factor for being overweight/obese in both boys and girls. However, long screen viewing times were associated with being overweight/obese only in boys.

ABSTRACT

Objective To study the effects of total lignans of Schisandra chinensis on memory impairment in rats with Alzheimer's disease, and to discuss its mechanism of action. Methods The rats were randomly into the blank control group, the sham injury group, the pathological model group, the low dose group of Schisandra chinensis and the high dose group of Schisandra chinensis with 15 rats in each group. The animals were given medicine at 3 days after the model establishment. The ainimals of the pathological model group, the low dose group of Schisandra chinensis and the high dose group of Schisandra chinensis established the AD model by D-gal intraperitoneal injection of compound Aβ hippocampal localization injection. The low dose group of Schisandra chinensis and the high dose group of Schisandra chinensis were administrated by means of intragastric administration 1mL physiological saline containing Schisandra chinensis total lignans 50 mg×kg-1×d-1 and 100 mg×kg-1×d-1, and the blank control group, the sham injury group, the pathological model group were administrated 0.9% Sodium Chloride Injection 1 ml×d-1by means of intragastric administration. After two weeks of continuous administration, the behavioral tests were carried out on the experimental animals, and the expressions of caspase-3, Bax, Bcl-2 and mRNA in the hippocampus of rats were measured by RT-PCR. Results Compared with the pathological model group, the escape latency shortened of the low dose group and the high dose group of Schisandra chinensis were significantly shortened (P<0.05). The number of traversing platforms (3.82 ± 1.23, 5.82 ± 1.18 vs. 2.13 ± 1.48), the percentage of swimming time (39.43% ± 7.12%, 48.63% ± 7.15% vs. 59.98% ± 6.13%) of the low dose group and the high dose group of Schisandra chinensis significantly decreased (P<0.05). Compared with the pathological model group, the expression of caspase-3(6.11 ± 1.25,4.42 ± 1.13 vs.7.88 ± 1.28)and Bax mRNA(5.84 ± 1.62,4.63 ± 1.51 vs.7.54 ± 2.14) significantly decreased (P<0.05), and the expression of the Bcl-2 mRNA(1.73 ± 0.82, 3.04 ± 1.29 vs. 0.93 ± 0.62) wese increased (P<0.05). Conclusions The diatomite Soxhlet extraction method is suitable for the isolation and purification of total lignans from Schisandra chinensis, and the total lignans of Schisandra chinensis can up-regulate the expression of Bcl-2, down-regulate the expression of caspase-3 and Bax, inhibit the apoptosis of neurons induced by A beta, improve the memory impairment of AD animal models, and the intervention effect is dose dependent.
